If you, or someone on your behalf, has asked Children, Young People and Families (CYPF) to help you with some difficulty which affects your child or your children an Initial Assessment is a brief process to find out if your child needs services, the nature of services required and when those services are to be delivered.

Childcare Questionnaire (pdf)
This questionnaire is entirely confidential and anonymous. The results are to help Birmingham Council find out the current childcare needs of parents of children up to 14 (or 16 for disabled children) including those doing shift work and work outside the normal working day or week. It is also to gain the views of parents about the cost and reliability of childcare. The overall results will be published by Birmingham City Council in the Three Yearly Assessment of Daycare in 2008.

If you have identified that a child/young person is in need of services you should complete a Common Assessment Framework, use this form if you are signposting or consider the child/young person is at risk. The completion of this form is not a pre-requisite of a referral. Urgent referrals (cases where children are in need of protection or immediate action is required to prevent significant harm) should be made directly, however the Framework for the Assessment of Children in Need requires that referrals are made in writing.
